SLED charges former Pee Dee Regional Center employee in vulnerable adult death

A former direct support professional at the Pee Dee Regional Center has been charged in connection with the death of a vulnerable adult earlier this year, according to the South Carolina Law Enforcement Division.

Haven Alyssa Jackson, 22, of Florence, is charged with abuse or neglect resulting in death of a vulnerable adult and abuse of a vulnerable adult. She was booked into the Florence County Detention Center following her arrest April 28.

According to arrest warrants, the charges stem from an incident on or about Feb. 1, 2026, at the Pee Dee Regional Center on National Cemetery Road in Florence County.

Investigators allege Jackson was working as a direct support professional responsible for the victim’s care and safety when the victim began choking. Despite being aware of the choking incident, Jackson is accused of failing to provide any life-saving assistance and not ensuring the victim received proper care, according to the warrant affidavit.

The warrant further states Jackson did not complete required documentation following the incident and failed to conduct a mandated 15-minute accountability check, actions investigators say contributed to the victim’s death.

In a separate charge, investigators allege Jackson physically abused the same victim multiple times throughout the day, including pushing the victim backward, causing falls, and using her body to block the victim’s movement and force compliance.

Authorities said the victim was a resident of the Pee Dee Regional Center and classified as a vulnerable adult. The allegations were supported by interviews and surveillance video, according to the warrants.

The Pee Dee Regional Center, which is operated by the South Carolina Department of Disabilities and Special Needs’ Office of Intellectual and Developmental Disabilities, requested the SLED investigation.

The case will be prosecuted by the 12th Circuit Solicitor’s Office.
